The Physical Therapist is responsible for supervising and participating in the application of a wide variety of physical therapy treatments to enhance rehabilitation of physical disabilities, injuries, and disease processes.

Job Responsibilities:

  • Performs comprehensive assessment at initial visit, re-evaluation visits, discharge and as needed;
  • Provides physical therapy services as prescribed by a physician in the plan of care, which can be safely provided in the home and assisting the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint and functional abilities test.
  • Develops and evaluates the patients plan of care in partnership with the patient, the patients representative (if any) and the patients caregivers

Education:

Graduated from an accredited institution after successful completion of a physical therapist education program

Experience

Minimum of one year as a physical therapist, home care experience preferred

Knowledge and Skills

Bilingual- English/Spanish, English/Russian and/or English/Creole preferred. Effective written and verbal communication skills. Familiarity with database management software programs and word processing. Ability to exercise initiative and independent judgment. Successful ability to negotiate conflict. Effective organizational skills with attention to accuracy, detail, follow-up. On-call availability. Own transportation and clean driving record. Must be able to be punctual and arrive at the patient’s home on time.
